SIR, Marshall et al. [1] propose pamidronate as a novel treatment for SAPHO syndrome (synovitis, acne, pustulosis, hyperostosis and osteitis) but we reported effective relief of pain for 3 months with a single infusion of 60 mg pamidronate in a patient under my care in a series of four patients in 1993 [2]. I prefer the term arthro-osteitis' to SAPHO syndrome as synovitis, acne and pustulosis are not sine qua non features of this rare condition. Pamidronate can be repeated safely when pain recurs, and it has become a useful treatment.
